Yugioh was originally not about the card game duel monsters. It was about games in general. You see, there's a boy named yugi who loves board games and stuff and is in high school. Because of his hobbies, he gets bullied a lot. Just watch the first episode of yugioh season 0 and you'll understand. Soon a man named ushio beats up the people who bully yugi, but yugi thinks this is going too far. Ushio than tells yugi that he wants dollars by the next day or he'll kill yugi using a dagger.
Yugi then solves a magical puzzle called the millennium puzzle and gets possessed by a pharaoh from ancient Egypt. The pharaoh challenges Ushio to a regular card game with poker cards. There were no duel monster cards cause like I said, originally, the show was about games in general. This was no ordinary game though, it was a shadow game, games where the loser may die, lose his soul, or become mentally ill or psychotic.
Usio cheats in the game causing the pharoh to give Ushio a penalty game. Ushio suddenly falls into the river where he gets swallowed by a giant man eating worm, it's just an illusion, but it causes Ushio to become mentally il and think he's dying. In the manga, which the show was based on unlike how pokemon was based on a videogame, Ushio, instead of thinking he's dying gets swallowed in his own greed and thinks everything, including leaves is money, making him still mentally ill.
Throughout the series, whenever yugi is in danger, the pharoh will posess his body and challenge the person putting yugi in danger to a shadow game. Once, the pharoh even set a loser to one of these games on fire, only in the manga though. Later on, the show does not focus on ga,es in general, but mainly a game called duel monsters. Some games of duel monsters are normal, while others are shadow game versions of duel monsters where the monsters will infact become real, and the players feel real damage instead of just seeing holograms of the monsters.
And the loser will most likely become mentally ill like what happens when you lose any shadow game. If you are interested in watching the original yugioh, watch it in japenese whith english subtitles because in English they censor violent scenes and people don't die. There's even a scene where a guy named Marik stabs his own father to death, and blood comes out.
